About the Job
The Beacon Case Manager will assist youth to transition into healthy adulthood by providing comprehensive case management support, mentorship, enrichment activities, information and referrals and other social services needed to support youth and their families who reside in the city of San Francisco and attend James Denman Middle School. The Beacon Case Manager’s responsibilities will depict monitoring cases by verifying students’ attendance and academic progress, tracking, and evaluating monthly case plan goals. In conjunction with the above effort, Beacon Case Manager will advocate for needed services and entitlements; obtaining additional resources; providing community resources and or city/county services to students and their families. The Beacon Case Manager also will maintain students’ records by reviewing case notes; logging coordination of services, resources disseminated, and overall progress or obstacles toward case plan goals.
Salary range: $23.00 to $28.00 per hour
Minimum Qualifications
- Minimum 21 years old
- AA degree or equivalent
- Must be available during program hours, Monday – Friday.
- At-least two years or more in the related field of social-work, sociology, youth development, & or psychology.
- Knowledge of best practices and principles of case management.
- Knowledge of the SFUSD and overall San Francisco community with an emphasis on low-income communities.
- Capacities to multi-task, work independently, and meet strict programmatic outcomes and deadlines.
- Proven ability to work with people, especially high-risk youth, from a variety of different ethnic, socioeconomic, educational, religious, sexual and generational backgrounds.
- Ability to carry out youth development practices in all program development and continually assess programs to ensure adherence to goals of program and organization.
- Computer experience with Windows, Word, Excel, database, internet and social media.
- Possess excellent written, verbal, interpersonal communication, as well as conflict mediations skills. Have a professional demeanor, pleasant personality, and a sense of humor.
- TB test clearance required
- Current YMCA approved CPR/FA certification required
Preferred Qualifications
- Bachelor’s degree or equivalent
- Three to five years of work experience in social-work, case management, youth development, and or trauma-informed practices.
