Position Summary

Reporting to the Associate Director of Transitional Services (ADTS), the Service Connector (SC) supports the Service Connector Program (SCP) at Potrero Hill through intake, outreach, referrals, and economic empowerment services. The role connects families with government agencies, community-based organizations, and property management; designs and facilitates community-responsive workshops; and collaborates with YMCA staff to provide a holistic, person-centered approach.

Key responsibilities include case management, accurate data collection and reporting in SCP systems, following program curriculum, coordinating incentives, conducting follow-ups, and participating in core events. Success in this role requires strong relationship-building skills and clear, respectful communication with families and youth, while maintaining confidentiality. Service Connectors are mandated reporters and must complete required training within 90 days of hire.

Job Responsibilities

  • Program Delivery & Family Support
  • Conduct family assessments and implement Family Service Plans using appropriate and supportive methods.
  • Plan, coordinate, and facilitate SCP workshops and events.
  • Build and sustain relationships within the YMCA and with community organizations to strengthen available supports.
  • Monitor program goals and outcomes (e.g., participant families reporting improved well-being; participants reporting positive, caring staff relationships).
  • Administration & Compliance
  • Complete and submit required documentation accurately and on time, as directed by the Site Manager and Associate Director.
  • Collect, enter, and report data according to program protocols.
  • Complete mandatory ADP LMS safety training and Y-USA e-Learning prior to first shift.
  • Adhere to YMCA policies on safety, supervision, mandated reporting, and risk management.
  • Community & Relationship Building
  • Communicate respectfully and effectively with staff, participants, families, and volunteers.
  • Participate in YMCA core events, community engagement, and outreach activities.

  • Attend staff meetings and trainings as required.

Qualifications

  • High school diploma or equivalent.
  • At least 1 year of case management experience with adults or families in individual and/or group settings.
  • Experience in interviewing, assessment, service planning, and working with community-based organizations.
  • Strong organizational, communication, and conflict resolution skills.
  • Ability to manage multiple tasks and follow through on commitments.
  • Availability to work on site at Potrero Hill (Mon–Fri, 8:30 am–5 pm or 10:30 am–7 pm, with some weekends as needed).
  • Ability to work respectfully and effectively with individuals and families from diverse backgrounds.
  • Professional, collaborative, and approachable demeanor.

Preferred Qualifications

  • Associate degree or equivalent experience.
  • Bilingual fluency in English and Spanish, or English and Cantonese (additional language skills are a plus).

Work Environment & Physical Demands

  • Ability to perform the essential functions of the job, with or without reasonable accommodation.
  • All employees working with the YMCA have a supervisory and safety responsibility when interacting with minors. Employment requires completion of a fingerprint background check, submitted to the Association Office Human Resources department before the first day of work.
  • Staff working on school campuses may be required to complete additional health and safety trainings

Salary Description

The Service Connector position offers a compensation of $25.00/hr. This rate is based on salary benchmarking for similar roles and is aligned with grant fund approvals and requirements for the position.
