About the Job

The CSD - Service Connector will be responsible implementing effective outreach strategies to assist with outreach and promotion of Service Connector Program services, activities, and events. SC will be responsible for screening a potential client to ensure they meet program requirements and conducting an intake for light case management services to adults 18 and up. The desired candidates will promote and implement programs that focus on housing stability, financial stability, workforce and educational opportunities to all low-income residents of SF City/County. We are seeking qualified candidates that can provide integrated, culturally relevant case management services, who holistic approach when working with individuals. Candidates will have demonstrated ability to: work toward health equity; impact health outcomes; support healthy communities using culturally responsive services; and work toward policy and systems level change. 

Minimum Qualifications

  • Minimum 21 years old
  • BA in Social Services field
  • Ability to complete accurate and comprehensive paperwork, client documentation and reports in a timely and efficient manner
  • Experience assessing clients' strengths and service needs for relevant and timely services congruent with client's capacity
  • Experience conducting formal intakes and needs assessments, preparing client service plans and providing post-service evaluations
  • Knowledge of the San Francisco and Hope SF communities and the support service network
  • Experience working collaboratively in partnership with non-profits, public agencies, educational entities, parents and other stakeholders
  • Familiarity with resources and support services and systems within San Francisco
  • Excellent organizational and communication skills with demonstrated follow through on tasks; multi-project/multi-task orientation
  • Ability to facilitate and/or coordinate workshops and community events
  • Experience complying with confidentiality policies and procedures and mandated reporting laws
  • Conflict resolution and problem-solving skills
  • Must be able to work 8:30 am - 5 pm or 10:30 am - 7 pm 3 times a week Monday-Friday, with at least 1 late day per week.  Some weekends as needed
  • Demonstrated ability to work with people from a variety of different ethnic, socioeconomic, educational, religious, sexual and generational backgrounds
  • Strong written and verbal communication skills
  • Comfort with traveling in San Francisco
  • Fluency in Spanish/English or Cantonese/English preferred
  • Background check clearance and TB test required
