Position Summary
Under the supervision of the Associate Director of Transitional Services (ADTS), the Service Connector (SC) plays a key role in supporting families within the Potrero Hill Public Housing community. This position focuses on fostering connections between families and vital resources, including government agencies, community-based organizations, and property management. The SC collaborates with YMCA staff to provide holistic support and is dedicated to building trust and strengthening the community through effective communication, case management, and culturally responsive programming.
The Service Connector is a part-time, temporary position with a planned end date of June 30, 2025.
Job Responsibilities
- Support the ongoing development and implementation of the Service Connector Program (SCP) at Potrero Hill Public Housing.
- Conduct intake, outreach, and follow-ups with families to provide appropriate information and support.
- Offer referrals to address family needs, including economic empowerment and targeted programming.
- Act as a bridge between families, government agencies, community-based organizations (CBOs), and the property manager.
- Collaborate with YMCA staff to deliver holistic support for families.
- Utilize case management approaches to effectively address family needs.
- Collect, document, and report program data in compliance with protocols.
- Manage program curriculum and incentives as provided.
- Participate in SCP core events and community activities.
- Maintain confidentiality and adhere to program guidelines and procedures.
Qualifications
- High school diploma or equivalent.
- At least 1 year of experience providing case management with adults or families in individual and group settings.
- Familiarity and experience in community-based agencies, client interviewing, assessment, and service plan development.
- Excellent organizational skills and demonstrated follow-through on tasks; multi-project/task orientation.
- Conflict resolution skills.
- Available to work on site 8:30 am-5:00 pm or 10:30 am-7:00 pm, Monday-Friday, and some weekends as needed.
- Proven ability to work with people from a variety of diverse backgrounds.
- Strong written and verbal communication skills.
Preferred Qualifications
- Associates degree or equivalent.
- Bilingual skills in Spanish or Cantonese.
